Here’s what you have to do:
1. Download and fill out the release forms found here
2. Send an email with your two (2) release forms and entry photo to:

Please submit one photo entry and have your entry in by May 14, 2010 at 5pm PST. If you're under 18, please have your legal guardian sign the forms as well. Submitting an entry does not guarantee that it will be used in the video.
